Quality Improvement Plan Form

The Quality Improvement Plan (QIP) form supports early learning program leaders in creating QIPs that summarize their program’s plan for advancing along Rhode Island’s quality continuum for early care and education programs. Programs can use this form to develop a QIP that meets requirements for Department of Human Services child care licensing and BrightStars participation.

The fillable QIP form is available in English and in Spanish. Once you open the form, be sure to follow the instructions. You can complete the form electronically and save the QIP to your computer.
