This pre-session assignment includes two tasks that will prepare you for our first session together. In Task 1, you will complete a survey that measures the family-provider relationship. In Task 2, you will complete a reflection worksheet about your experience taking the survey.
The estimated time to complete this assignment is 1 hour. You will receive credit for this time on the certificate of completion issued at the end of the series.

Task 1: Complete Survey
The Family and Provider/Teacher Relationship Quality survey is a series of measures designed to assess the quality of the relationship between families and providers/teachers of early care and education for children birth to 5 years of age.
In this task, you will print and complete the survey appropriate to your role. The survey takes about 10 minutes to complete and asks general questions about the education and care environment and about your current practices for communicating with and engaging families. Please bring the completed survey with you to Session 1 to receive full credit for the assignment.
- If you are a program administrator, director, or education coordinator, please print and complete this survey.
- If you are a teacher or family child care provider, please print and complete this survey. (Note: You do not need to respond to the background questions on the last two pages, numbers 13–16.)
Task 2: Reflection Worksheet
In this task, you will complete the Assignment Reflection Worksheet. Please bring the completed and printed worksheet to Session 1. Give the worksheet to your instructor to receive full credit for the assignment.
